Palliative Care Social Work Clinician
The Palliative Care Social Work Clinician coordinates the development and implementation of a plan of care as desired by the client and family. The Palliative Care Social Work Clinician completes bi-psycho-social assessments and provides appropriate social work and care coordination interventions. The Palliative Care Social Work Clinician has primary responsibility for bereavement and grief group process management. Palliative Care Social Work Clinician is involved in volunteer training and placement, and community-based and professional education in a support role.
Master’s degree in social work. LCSW preferred. If not LCSW, the ability to get LCSW within 4 years of hire. Supervision during work hours by Clinical Director is available.
Flexible schedule -- between 30 and 40 hours, depending on the candidate's needs/desire. $26-$27/hour DOE.
