Housing Case Management Specialist
Full-time position providing community-based dual diagnosis support services to adults that experience mental illness and are in the transition from institutionalization or homelessness to independent living. This position requires flexible hours to include evenings and weekends, and 75% of services are provided in the community.
Primary duties include contributing to assessment and service planning, individual and group skill building, crisis management, rehab services, and other support services. Minimum of bachelor's degree with course study emphasis in behavioral/emotional or physical disabilities or five years' experience working with mentally ill clients preferred. Relevant experience may be substituted for educational requirements. Substance abuse treatment experience preferred. Knowledge of group dynamics and commitment to client-centered work a plus.
Scheduling may include evenings, nights, weekends, and holidays. Salary range is $19.35 to $25.04/Hour.
